Registered Nurse (RN) – Clinical Liaison & Business Development

Position Summary Veridia Home Care is seeking an outgoing, motivated, and compassionate Registered Nurse (RN) to join our team as an RN Clinical Liaison & Business Development Representative serving the Winston-Salem, Greensboro, and surrounding areas. This field-based position serves as a bridge between skilled nursing facilities, patients and families, referral sources, and our home care team. The ideal candidate will combine clinical knowledge with strong communication and relationship-building skills to support successful transitions from facility to home while developing and maintaining referral relationships throughout the community. Key Responsibilities • Meet with prospective patients and families prior to discharge to introduce Veridia Home Care, explain available services, and help prepare for a smooth transition home. • Conduct post-discharge follow-up with patients and families to ensure services are going well and identify additional needs or concerns. • Develop and maintain relationships with social workers, discharge planners, case managers, skilled nursing facilities, assisted living facilities, rehabilitation centers, and other referral sources. • Conduct regular facility visits and community outreach to support business development and referral growth. • Follow up on potential referrals and help coordinate the process from initial referral through start of care. • Review prospective patients from a clinical perspective and communicate relevant care needs to the appropriate agency staff. • Work closely with the Director of Operations and clinical team to coordinate patient transitions and follow-up. • Identify new referral opportunities and strengthen existing facility relationships. • Maintain timely and accurate documentation of patient/family interactions, referral activity, and follow-up. • Represent Veridia Home Care professionally within facilities and throughout the community. • Provide nursing and clinical support as appropriate and within scope of practice. Qualifications • Current and unrestricted North Carolina RN license. • Previous nursing experience; home care, skilled nursing, assisted living, rehabilitation, or long-term care experience preferred. • Strong interpersonal, communication, and relationship-building skills. • Comfortable communicating with patients, families, social workers, case managers, and facility leadership. • Outgoing and comfortable with community outreach and business development. • Strong organizational and follow-up skills. • Ability to work independently and manage multiple referrals and relationships. • Valid driver’s license and reliable transportation. • Current CPR/BLS certification preferred.
