OnEarly is the next act from the team behind OneMRI, Australia’s largest full-body MRI provider, scanning tens of thousands of Australians across a network of 60+ radiology practices.
Founded by Gopi Sara, Dr.
Vu Tran, and Andrew Barnes, our mission is to transform health outcomes by delivering 10 million diagnostic scans a year.
Now, we are building the unified ordering layer for healthcare testing.
Practitioners use OnEarly to seamlessly order tests, while consumers use it to discover, book, and access their results.
Medical testing represents a $10B+ market in Australia, and no single platform owns this infrastructure yet.
You’ll be joining our engineering team at the foundational ground floor of this build.
About the Role
You’ll join a tight-knit engineering crew building the core marketplace platform connecting practitioners, pathology and radiology providers, and consumers.
You will take ownership of fundamental marketplace capabilities, from the practitioner ordering flow and patient booking experience to supply-side integration layers and internal operational tooling, much of which is being designed from scratch.
While early parts of our operations are high-touch, your primary focus is helping design and build the digital engine that scales both sides of the network.
You’ll work closely with operations, clinical partners, and end-users, giving you full visibility into real marketplace dynamics and business needs before they hit a formal specification.
This role spans a broad technical surface area across the entire product.
You will own features end-to-end, from initial design and architecture to production deployments and operational monitoring.
Your product intuition and architectural judgment will matter as much as your execution speed.

What You’ll Do
Ship with AI-native development workflows: Use AI coding agents (scaffolding, refactoring, test generation, migrations) as a core operational multiplier, maintaining a high bar for code quality through rigorous review.Build across web and mobile surfaces: Design, build, and maintain practitioner and consumer features across web and mobile using React, React Native, and TypeScript.
Extend a shared design system and compose features from reusable components.Backend integrations, APIs & relational schemas: Develop secure, reliable backend services and REST/GraphQL APIs using cloud-managed distributed and event-driven patterns.
Design relational data models using PostgreSQL to handle test ordering, results pipelines, and integrations with radiology and pathology networks.Operate on cloud infrastructure: Deploy, manage, and scale cloud infrastructure on AWS, taking ownership of CI/CD pipelines, deployments, and production observability.Automate operational workflows: Work directly with ops and partners to replace manual processes with software and internal tooling, deciding what to automate first based on real pain points.Drive end-to-end engineering rigor: Write clean, maintainable, and well-tested code.
Participate in code reviews, investigate production issues, write clear technical documentation, and establish system architecture practices that scale smoothly.Bake in compliance from day one: Build with health privacy, consent management, auditability, and data security standards baked directly into the system design.
What We’re Looking For
Proven AI-Augmented Workflow: Real-world experience building software where AI coding tools are central to your daily workflow.
You can articulate where agents accelerate output and where manual oversight and deep engineering judgment are required.Deep CS & Systems Fundamentals: Strong grasp of relational data modeling (PostgreSQL), distributed system failure modes, consistency models, and API design.
You spot edge cases early and design resilient systems.Observability as a Baseline: Hands-on experience with instrumentation and observability platforms like Graphana/Datadog.Comfort with Ambiguity: Thriving in early-stage environments where specs are lightweight, requirements evolve, and your work directly steers product direction.
End-to-End Ownership: Demonstrated experience owning systems in production, including deployment pipelines, monitoring, and operational readiness.Clear Communication: Ability to write concise technical documentation and communicate technical trade-offs effectively to non-engineering founders and internal stakeholders.Regulatory & Health Data Aptitude: Aptitude for (or experience with) order lifecycle management and data privacy standards especially regarding health information (Australian Privacy Act minimum but US and UK in addition is ideal).
