Senior Full Stack Healthcare Developer
Role Summary:
We are seeking a Senior Full Stack Software Developer with strong hands-on experience building modern healthcare applications using Java, Spring Boot, React, TypeScript, FHIR, SMART on FHIR, and REST APIs.
The successful candidate will contribute to the modernization of healthcare provider and service discovery applications supporting eReferral workflows within Electronic Medical Record (EMR) environments.
The role requires deep experience with healthcare interoperability, embedded SMART on FHIR applications, FHIRCast-based context synchronization, advanced search, identity and access management, and event-driven healthcare integrations.
This position is best suited for a senior engineer who can work across both front-end and back-end development while understanding complex healthcare interoperability requirements and enterprise integration patterns.
Key Responsibilities:
Design, develop, maintain, and support Spring Boot microservices and modern React/TypeScript user interfaces supporting healthcare provider and service discovery.Develop modern React applications using component-based architecture, state management, reusable design patterns, automated testing, performance optimization, accessibility, and maintainable UX practices.Implement and support SMART on FHIR applications embedded within EMRs, including SMART launch, launch context, fhirContext, OAuth2/OIDC authorization, and FHIR read/write operations.Design and implement FHIR R4/R4B and REST APIs supporting providers, practitioners, organizations, healthcare services, locations, and eReferral workflows.Implement healthcare context synchronization using FHIRCast, including patient, user, encounter, and application context management and support for applicable Ontario CMS standards.Implement event-driven healthcare integration using FHIR Pub/Sub, FHIR Subscription, or comparable messaging and event-driven patterns.Design and develop advanced provider and healthcare service search capabilities using OpenSearch or Elasticsearch, including semantic search, relevance tuning, filtering, faceted search, autocomplete, and intelligent search suggestions.Develop advanced Google Maps Platform functionality for geographic healthcare service discovery, including geographic filtering, regional and polygon overlays, clustering, and interactive map visualizations.Develop Spring Boot services that enforce user roles, permissions, entitlements, organization access, and dataset-level access controls when searching and retrieving information.Integrate healthcare applications with NetIQ using OAuth 2.0, OpenID Connect, JWT, and role-based access control.Integrate with IBM Master Data Management (MDM) or comparable enterprise master data platforms to consume and synchronize provider, organization, service, and location information.Develop data transformation, validation, synchronization, and search-indexing services across multiple healthcare and enterprise datasets.Design, develop, secure, and maintain RESTful APIs, FHIR APIs, and distributed microservices.Implement and maintain integrations with AWS services such as API Gateway, ECS, Lambda, S3, EventBridge, SNS, SQS, IAM, CloudWatch, and Secrets Manager as required.Develop automated unit, integration, API, and UI tests and support CI/CD pipelines and automated deployments.Troubleshoot application, integration, API, search, security, and production issues and implement corrective solutions.Participate in architecture reviews, technical design sessions, sprint planning, backlog refinement, peer reviews, and collaborative troubleshooting.Maintain technical architecture documentation, API specifications, configuration documentation, deployment guides, operational procedures, and knowledge-transfer materials.Support production releases, monitoring, operational support, and knowledge transfer to the delivery team.
Must-Have Requirements:
SMART on FHIR & Healthcare Interoperability-
Strong hands-on experience developing SMART on FHIR applications embedded within EMRs.Experience with SMART launch, launch context, fhirContext, OAuth2/OIDC authorization, and FHIR read/write operations.Hands-on experience implementing FHIR R4/R4B integrations and healthcare interoperability workflows.Experience implementing FHIR Pub/Sub, FHIR Subscription, or equivalent event-driven healthcare integration patterns.
FHIR & REST APIs-
Strong experience designing and developing FHIR R4/R4B and REST APIs.Experience supporting healthcare provider, practitioner, organization, healthcare service, location, and eReferral workflows.Experience designing, securing, developing, and supporting APIs within distributed enterprise environments.
FHIRCast / Context Management-
Experience implementing FHIRCast-based context synchronization or equivalent healthcare context management patterns.Experience supporting patient, user, encounter, and application context synchronization.Experience with Ontario CMS standards or comparable healthcare context-management standards.
Search & Provider/Service Discovery-
Strong experience implementing advanced provider and healthcare service search using OpenSearch or Elasticsearch.Experience with semantic search, relevance tuning, filtering, autocomplete, intelligent suggestions, faceted search, and search optimization.
Identity & Access Management-
Experience integrating applications with NetIQ or comparable enterprise IAM platforms.Strong knowledge of OAuth 2.0, OpenID Connect, JWT, authentication, authorization, and role-based access control.
Public Sector Experience-
Public sector experience is mandatory.Experience working within large, complex public-sector or healthcare technology environments.
