Founding Engineer, Applied AI

Location: Berlin Mitte (Hybrid – 3 days/week in office) + Occasional customer visits Compensation: €60,000 – €90,000 + Bonus + VSOP Equity Experience Level: 3+ years Language Requirement: B2 German (Conversational proficiency required) About the Role & Mission Doctors currently spend up to 68% of their day on administrative work and paperwork instead of caring for patients. We are an early-stage, mission-driven HealthTech startup on a mission to build AI agents that eliminate this burden, giving physicians their time back so patients get the focus they deserve. As our Founding Engineer (Applied AI), you will be our second hire in engineering, working directly alongside the CTO. This is a high-agency, founder-level role where you will own core backend systems end-to-end and ship code directly into clinical environments within weeks of joining. Key Responsibilities Own Backend & Infrastructure: Develop and scale the Python backend powering our real-time medical documentation assistant in production environments across Germany.Build Real-Time Audio Pipelines: Implement live transcription pipelines utilizing streaming audio, multi-provider Speech-to-Text (STT), quality monitoring, and health-aware failover architectures.Optimize Applied AI Architecture: Design and manage our LLM evaluation pipelines, prompt optimization strategies, and multi-provider routing/fallback chains while balancing cost, speed, and accuracy at scale.Personalize the Medical AI: Analyze LLM traces and doctor edits to construct memory systems that tailor documentation preferences to individual medical practices.Lead Greenfield Initiatives: Drive new features and product generation from initial concept to deployment, taking full ownership of multi-cloud production infrastructure, Docker, and CI/CD pipelines.Direct Clinical Engagement: Communicate technical updates to doctors 1–2 times per week to gather direct feedback and iterate on product mechanics. What We Are Looking For Technical Depth: 3+ years of professional experience writing clean Python, shipping production-grade systems, and working with modern Applied AI/LLM architectures (evals, routing, prompting, real-time pipelines).High Agency & Co-Founder Mentality: You take pride in building and shipping end-to-end solutions that people depend on—not just ticking off assigned engineering tasks.Tech Stack Familiarity: Hands-on experience with FastAPI, real-time WebSocket audio streaming, Supabase/PostgreSQL, Docker, CI/CD, and multi-cloud infrastructure.Communication & Language: Conversational German (B2 minimum) to engage with German medical documentation context and occasionally speak directly with users.Location: Readiness to work 3 days a week hybrid from our Berlin Mitte office, with occasional travel to medical practices across Germany. Nice-to-Haves: Experience with Dart / Flutter (Windows Desktop & Web).Prior experience in MedTech, healthcare, or MINT/CS backgrounds.A strong visual or product eye for frontend polish.
